Pharmacy Errors Are Common

Medication errors take place on a daily basis in the United States. As little as three percent and as many as 15 percent of all prescriptions are filled in error, posing a risk of injury or death to the patient. Medication errors are so common that they account for about 25% of all medical malpractice cases. Moreover, medication errors are estimated to cause about 7,000 deaths per year.

Causes of Medication Errors

There are many causes of medication errors. Illegible handwriting on the part of the physician can be a factor in some medication error cases, but the majority of pharmacy errors are due to the conduct of the pharmacy staff. Medication errors can be avoided through proper education, training, management and technology, but many large pharmacies are reluctant to take preventative actions that may reduce profit for the company. Accordingly, pharmacy errors continue to be a major problem for patients simply trying to fill a prescription.
